Voor onze huisartsenpraktijk De Graeff in het mooie medische centrum De Graeff in Purmerend zijn wij op zoek naar een Praktijkondersteuner huisartsenzorg somatiek (POH-S) voor 24 uur per week. Binnen onze praktijk werken we samen met meerdere POH-S collega’s in een gezellige en dynamische omgeving. Doordat één van onze POH-S collega’s minder gaat werken, zijn wij op zoek naar versterking. Wie zoeken wij?
•
Je bent een enthousiaste, flexibele collega die graag nieuwe dingen leert.
•
Je bent communicatief vaardig en kan zowel goed in een team als zelfstandig werken.
•
Je hebt bij voorkeur ervaring met begeleiding van risicogroepen zoals patiënten met diabetes, COPD, astma en een hoog cardiovasculair risico, of bent bereid dit te leren.
•
Je bent beschikbaar voor 24 uur per week.
•
Je bent opgeleid als POH.
